Please read this carefully before going to next page... 12th Annual Golf Tournament is held in Hood River, Oregon at Indian Creek Golf Course at 8:00 am Saturday June 28th 2008 4 Person Scramble--Shotgun Start The registration form must be printed and filled out and sent by US Mail only! Make checks payable to Benefit Golf Classic Send printed copy of registration and check to: PMOC C/O Gene Daily 7544 NE 33rd Dr Portland, OR 97211 Entry fee is $75.00 per person (non-tax deductible). This includes unlimited range balls, green fees, cart, lunch and door prizes. Foursome pairings will be selected randomly. If you have any questions please call. The golf course is limited to only 132 golfers. Only the first paid and completed registrations post marked May 23th 2008 or later will be accepted. Please be ready to golf at 7:30 am and remember, that this tournament is for 18 years and older Please sign up in pairs. If you elect to sign up as a Foursome, your team will not be eligible for prizes.
